The formdatabase.com MCP server lets an agent list forms, email a magic link, check status, and transfer a finished form into QC Database. You sign in with your QC Database account, and the agent can only do what you can.
Agent clients that support remote MCP register themselves, then open a browser so you can approve access. You do not create a client secret or paste an API key.
Your account
You can add the server while signed out. The agent will ask you to sign in with QC Database the first time it calls a tool.
On a Pro or Max plan: Customize → Connectors → Add → Add custom connector. Name it, paste the server URL, and choose sign-in (OAuth).
On Team or Enterprise, an owner adds it first under Organization settings → Connectors → Add → Custom → Web. Each person then connects it from Customize → Connectors.
Approve the QC Database sign-in when Claude asks. You do not paste an API key.
ChatGPT
In ChatGPT on the web, open Plugins or Apps & Connectors (the name depends on your plan) and choose Add custom MCP server.
Enter a name and the server URL. Choose OAuth if asked how to authenticate.
Create the connector, install it, and sign in with QC Database when the browser prompt appears.
Claude Code
From a terminal, add the remote server with the command below.
In Claude Code, run /mcp and complete the QC Database sign-in in the browser.
Terminal
claude mcp add --transport http formdatabase-com https://mcp.formdatabase.com/mcp
What an agent can do here
The person who fills the form needs an email and no QC Database account. The agent sends the link; it does not fill the form as them.
A tenant admin adds this app under Tenant Settings → Approved MCP and Integration origins before an agent can sign in.
This app isn't enabled for your organization yet. Ask your tenant admin to add formdatabase.com and mcp.formdatabase.com to Approved MCP and Integration origins.
